We offer an advanced booking appointments system and same day appointments.
Each 10min appointment is for one patient with one medical problem only. Please make a separate appointment for each member of your family or if you have more than one medical problem.
Some of our clinicians consult in consulting rooms on the first floor at Osmaston Surgery. If you cannot manage to climb the stairs, please mention this to the receptionist when you book your appointment so that we can make arrangements for the consultation to take place in a downstairs consulting room.

When you get in touch, we’ll ask what you need help with.
We will use your answers to choose the most suitable doctor, nurse or healthcare professional to help you.
On the day appointments
On the day appointments are available to book from 8am. The earlier you ring, the more choice of appointment times you will have.

Extended hours service
Derby City Primary Care Ltd comprises of 4 GP surgeries across Derby City, including:
- Horizon Healthcare
- Osmaston Surgery
- Derwent Medical Centre
- Wilson Street Surgery
We operate 6 days a week:
- Monday to Friday, 5pm to 8pm
- Saturdays, 9am to 1pm
Cancelling or changing an appointment
If you are unable to attend your booked appointment, please let us know in advance so we can offer the appointment to someone else.
If you are late for your appointment, you may be asked to re-book another appointment or you may have to sit and wait until the end of surgery before you are able to be seen. Patients who arrive after you, but who are on time for their appointment, will be given precedence.

Failure to attend appointments
Repeated non-attendance is unfair on our other patients. If you fail to attend an appointment and don’t let us know within a timescale that would allow someone else to use the appointment this will be recorded in your medical records.
If you repeatedly fail to attend appointments your GP will write to you warning that this is unacceptable and if it continues you will be asked to leave our practice list.
If you need help when we are closed
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
This service is managed by Derbyshire Health United (DHU) which is a co-operative of qualified GPs from the Derby area based within the out of hours centre.
A health care professional will answer your call and ask you for details of your problem. You may be offered telephone advice or asked to attend the out of hours centre to see one of the emergency care practitioners or a doctor/nurse. If patients are not well enough to travel and a home visit is deemed necessary this can still be offered. Patients should not arrive at the DHU centre without first telephoning for advice.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

- if there’s a specific doctor, nurse or other health professional you would prefer to respond
- if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
- if you need an interpreter
- if you have any other access or communication needs
Home visits
Due to ever increasing pressures on the doctor’s time a home visit may not always be offered automatically. If possible, please try to phone reception on 01332 346433 before 10:15am if you require a home visit. You will be asked to give a brief description of the problem. This will give the GP time to look through the visit requests and prioritise each one.
A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.
Home visits are only available for patients who are housebound because of illness or disability.
Children who are feeling poorly can safely be brought to the surgery where they can be more fully assessed on our well equipped premises.
Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the surgery.
